In order to participate in athletics at Einstein, student-athletes must also meet the following requirements as described below:
Annually
Athletic Contract | Signed by both parent/guardian and student. |
Sports Physical Form | Completed and turned in before the athlete can participate. |
Concussion Information | Reviewed and signed by both parent/guardian and student. |
Middle School Participation Form: | Completely filled out and signed by parent or guardian. Insurance is required in order to participate in any sports. |
Sudden Cardiac Arrest: | Reviewed and signed by parent or guardian and student athlete. |
ASB Card: | $25 cash/check made payable to Einstein Middle School. |
EACH Sports Season
Sport Specific Risk Management Form | Signed by athlete and parent/guardian. Individual coaches will collect these forms. Do not turn in until coach instructs you to do so. |
Athletic Participation Fee: | ($20 per sport) Cash or checks made payable to Einstein Middle School. Football, Cross Country, Wrestling, and Track are all non-cut sports and the fee is $20. Girls Softball, Girls Volleyball, Girls and Boys Basketball are all cut sports. For “cut” sports: students will try out for these teams and if they are chosen for the team will pay the $20 fee. There is no fee to try out. Upon making the team, the $20 fee will be due prior to the first contest. |
- Scholarships are available for students needing financial assistance. Please contact a school administrator, counselor, or coach.
- No refunds will be approved for students who are dropped for athletic code violations or who quit after the first interscholastic contest.
- For “cut” sports: students will try out for these teams and if they are chosen for the team will pay the $20 fee. There is no fee to try out. Upon making the team, the $20 fee will be due prior to the first contest.
General Expectations
The following expectations apply to all student-athletes throughout each season and will be carefully considered by coaches when making decisions.
Academics: | All athletes must pass 5 classes the previous trimester to be eligible to turn out. During the season athletes must be passing all classes to remain eligible for competition. |
Attendance: | Attendance at practices and games is instrumental in the development of teamwork, skills and knowledge of the sport. Athletes must attend a minimum of 50% of the school day to practice or compete. |
Skill/Physical Fitness: | Athletes should demonstrate teamwork, sport specific skills and on-going effort towards maintaining high levels of physical fitness. |
Citizenship: | Athletes will demonstrate positive interpersonal, social skills and sportsmanship. Athletes are expected to follow all school, classroom and team rules, respect other students and staff. |
Sportsmanship: | Athletes should demonstrate a positive attitude, be encouraging, respect their own team as well as the opposing team, referees and game officials. |
