2016-2017 Supply List – Class Fees need to be paid to Ms. Warfield in the business office (just down the hall from the main office, the window on the left). The business office is open for students to pay fees before school, the 2nd half of their lunch, or immediately after school dismissal. Acceptable forms of payments are cash or checks/money orders payable to Kellogg Middle School. School lunch account payments must be made with a separate check paid directly to the cafeteria. Please make sure to include your students name in the memo/note line of any checks.

COLLEGE BOUND – Counselors are also available to help parents with more information about College Bound Scholarships (available to students that qualify for free or reduced lunch).

Immunization Exemption: The State Legislature made some adjustment this past session to how students are exempted from immunizations. All students are required to be immunized unless they have a valid Certificate of Exemption. You may download the Certificate of Exemption here.
