Daily Health Attestation Forms
The most significant aspect of maintaining the health and safety of students and staff at school is to educate our greater Shoreline community about the importance of staying home if they are sick or if they have been in close contact with someone who has confirmed or suspected COVID-19 in the last 14 days.
Each day, staff and parents/guardians are expected to assess whether they or their students are exhibiting any symptoms of COVID-19 or have been in close contact with someone who has been confirmed or is suspected to be positive for the virus. This daily assessment serves the purpose of protecting our school community, and reminding each of us of our part in reducing the spread of the virus. Any student or staff member must not come to school or work if they have symptoms of COVID-19 and/or they have been in close contact with someone who has been confirmed or suspected positive with COVID-19 in the last 14 days.
The regular expectation will be for each staff member and student to complete a health screening PRIOR to coming to school each day.
This is called an “attestation,” and it will be accomplished at home via ParentSquare, the District's new communication platform, or by paper copy.
