Shoreline School District Immunization Requirements 2020 - 21
Washington State law requires that all children up to the age of 21 be fully immunized on or before the first day of school. Students must have proof of immunization or an approved Certificate of Exemption in order to attend school.
This means the school MUST have on file:
- A Certificate of Immunization form, OR
- A Certificate of Exemption Form** exempting the student from these requirements for medical, philosophical or religious reasons.
**Requirement for Certificate of Exemption **
The Certificate of Exemption form for newly enrolled students and current students who have new immunization requirements must include a healthcare provider’s signature for those wishing to exempt for medical, religious or philosophical reasons. A religious membership exemption is used when a parent/guardian affirms membership in a church or religious body that does not allow medical treatment for the child. A healthcare provider’s signature is not required only in this case.
Personal exemptions are not allowed for the MMR vaccines for any students or children in childcare. Medical and religious exemptions are still allowed. Information about this is available here.
***UPDATE FOR IMMUNIZATION COMPLIANCE DURING ON-LINE LEARNING
All immunization documentation should be turned in to your school by the first day of attendance (September 2nd for Grades 1-12 and September 8th for kindergarten and preschool). Valid documentation includes medical records showing vaccination, evidence of immunity to the disease the vaccine prevents, or a completed Certificate of Exemption (COE) form.
For this school year:
- Shoreline School District WILL NOT exclude any student from ON-LINE learning activities for missing immunizations or exemption paperwork.
- Shoreline School District WILL exclude students from IN-PERSON learning for missing immunizations or exemption paperwork. In-person learning includes being on campus for any reason, including childcare, small group work or extracurricular activities.
