This paper form is only to be used if unable to complete the Chromebook agreement electronically via Skyward Family Access.
This form is to be used to request a waiver of the Chromebook program fee. It must be signed by a parent/guardian and returned to the school tech office.
Use this form if you wish to opt your student out of the high school Chromebook take-home program. District Chromebook will still be required for in school use. Student will be responsible for checking device out and back in each school day.
