• New Immunization Rules coming August 2020

    1. Starting for students enrolling in August 2020, all immunizations submitted to schools must be medically-verified. Examples of medically-verified immunization records include:
      • A Certificate of Immunization printed from the Washington State Immunization Information System
      • A medical record from a healthcare provider with their name and signature
      • A medical record with an official health facility or provider logo/stamp (for example, a booklet that is stamped with the date, vaccine and clinic)
      • A document from another US state or territory immunization registry

Washington State Immunization Information System (WAIIS)

Many, but not all, medical providers enter vaccine dates into the statewide database. School nurses or assigned staff may view and search these records for missing immunizations. Your signature on the Certificate of Immunization Status (CIS) form allows us to enter these dates onto your child’s school record.

Parents can print their own child’s Certificate of Immunization Status (CIS) through a new web portal called MyIR. This secure online system pulls data from the WA Immunization Information System (WAIIS) and will auto populate the CIS with your child's immunizations.  Parents just need to print, sign, and submit the CIS to the school or preschool/child care. Go to this website for more information.

    **Requirements for Certificate of Exemption**

    • The Certificate of Exemption form for newly enrolled students and current students who have new immunization requirements must include a physician's signature for those wishing to exempt for medical or philosophical reasons.  A proof of membership in a religious group that does not allow a healthcare practitioner to provide medical treatment to a child is required for those claiming a religious exemption.
    • As of July 2019, WA state no longer allows a personal or philosophical exemption for the MMR vaccine. This does not include medical or religious exemption. Please talk with your healthcare provider if you have questions or concerns.
