Free & Reduced Program

The cover letter/parent release form is to be filled out if parents are interested in fee reduction/scholarships available at their student's school; such as field trips, lap top fees, class fees, ASB fees, College entrance exam fees, etc. Each school has different scholarships available so parents need to talk to their student's school about these opportunities. The release would be filled it out and given it to the school nurse. It gives permission to the school nurse to access information about the student's eligibility for free/reduced priced meals.

Exhibit II contains information about meal prices, USDA income guidelines, and other information about the application to be filled out.

• Letter to Households

The Free/Reduced-priced meal application is Exhibit IIA, pages 1 and 2, both pages need to be filled out and returned to us for processing.
